Scheduled appointments

Please arrive 10 to 15 minutes prior to your scheduled appointment time. Should you need to cancel or reschedule your appointment, please contact our office as soon as possible.

Preparing for your appointment

Please bring the following items to your appointment:
-Government issued ID / Passport / Social Security Card
-Insurance card or a copy of your insurance information
-A list of all medications you are currently taking or have taken recently, whether over-the-counter or prescription. Please include the strength and dosage of each medication.

Workers' Compensation

If your treatment is covered by Workers' Compensation, please bring the following: -Employers' letter of authorization to treat the patient -Workers' Compensation form completed by your employer -Government-issued ID card -Workers' Compensation Claim Number


Our office is "in-network" with most insurance plans. The managed care health insurance market changes rapidly, any specific coverage questions should be discussed directly with your insurance company or our knowledgeable staff. You can always call us for inquiries.
